Abstract

Anaphylactoid reactions have been described after infusion of HES as well as HSA, the incidence being 0,08% for HES and 0,01% for HSA reactions. Contrary to other colloids late reactions have been observed after HSA infusion occurring after several days of high-dose HSA treatment. A remarkable feature of HES incompatibility is a prolonged local reaction along the vein infused. In the pathogenesis of HSA reactions protein aggregates seem to play a role in some patients. Stabilizers used during the production of HSA or plasmaprotein solutions can change the immunogenicity of HSA and might be of possible pathogenic importance. In some patients suffering from HES incompatibility changes in plasma complement levels were observed. Anti-HES-antibodies have been demonstrated in low titers in human volunteers as well as in surgical patients. So far no correlation between anti-HES-antibodies and anaphylactoid reactions has been found.
