Abstract
Background: Chronic instability of the acromioclavicular joint (ACJ) can lead to dyskinesia of the shoulder joint, rotator cuff dysfunction and chronic pain. There are a variety of surgical techniques described for treatment including anatomical and non-anatomical repair, biological augmentation and synthetic graft augmentation. Purpose: This article gives an overview of the various treatment options and provides a standard surgical technique for the treatment of such injuries Material and methods: A narrative review of the published studies with respect to the usual surgical techniques and outcomes. Results: Biological augmentation with a suture button fixation has biomechanical advantages over the ligament transfer and synthetic graft procedures. It also leads to more predictable clinical and radiological outcomes. Conclusion: The use of biological augmentation using a free tendon graft along with a suture button fixation by an arthroscopically assisted technique is the current standard treatment for the management of chronic ACJ injuries.
