Abstract

Confrmation of food-dependent exercise-induced anaphylaxis (FDEIA) is challenging. Typically, the allergenic foods are well tolerated in absence of triggers. Furthermore, besides physical exercise there are a variety of other potential co-factors that can induce severe allergic reactions together with culprit foods in sensitized patients. The considerable variability of inter-and intraindividual relevant allergen sources, eliciting allergen doses and co-factors complicates oral food challenges in patients with FDEIA and produces unreliable results. The best known entity of FDEIA is wheat-dependent exercise-induced anaphylaxis (WDEIA). In this article, the diagnostic work-up for WDEIA is reviewed and discussed.
