Abstract

The aim of established oncological surgery is tumour resection with curative intention. Indeed minor access surgery is feasible in many oncological situations, but because of tumoursize, tumourlocalization and obligatory lymph node dissection, standards of conventional oncological surgery often cannot be guaranteed. Therefore laparoscopic surgery has not yet gained an established place in oncological therapy. An exception could be laparoscopic surgery of the left hemicolon and the rectum, where the evaluation of laparoscopic techniques in comparison to conventional techniques under strict study conditions seems to be justifiable. Also in case of early carcinoma of the stomach of the mucosa-type (if histologically confirmed) the evaluation of limited procedures is admissible. Currently the use of minor access surgery in all other oncological situations is still not justified. However, laparoscopic and endoscopic procedures could become important pillars for diagnostic work-up and for palliative oncological surgery.
