Abstract
The morbidity of gastric resections amounted to about 30%, even in medical centers, with cardiopulmonal complications and insufficiency of the proximal anastomosis being the most frequent causes of morbidity. General complications are avoided most effectively by systematic preoperative risk assessment and consistent postoperative management. Fast and systematic diagnosis in case of any disturbance of the postoperative course is the key factor for the successful management of anastomosis insufficiency. This implies an immediate and forceful intervention if a insufficiency is proven to limit contamination und to minimize postoperative sepsis. The close and fruitful collaboration of the surgeon with intensive care specialists, endoscopists and interventional radiologists as well as the acceptance even of the experienced surgeon that complications may occur are indispensable for the successful management of these complications.
