Abstract
Tap water iontophoresis is an established therapy for hyperhidrosis. Some recent clinical investigations showed therapeutic efficacy also in dyshidrotic eczema in two uncontrolled studies, dyshidrotic eczema in patients with hyperhidrosis cleared simultaneously to normalization of sweat production under tap water iontophoresis. In our own randomized half-side study dyshidrotic hand eczema significantly improved under tap water iontophoresis regardless of hyperhidrosis. The therapeutic mechanism is unclear; presumably iontophoresis besides postsynaptic functional modifications of sweat gland activity also has neuropeptidergic antiinflammatory effects. Tap water iontophoresis with pulsed direct current has low side effects and can be of benefit in maintenance therapy of chronic dyshidrotic eczema.
