Abstract
Summary We report about a 20-year old man, who suffered from a car accident under the influence of ecstasy. Although he had no visible injuries at the chest, the accident caused an isolated rupture of a papillary muscle of the mitral valve. A review of the literature shows the rarity of this kind of injury. The mechanisms of traumatic papillary muscle rupture, the diagnosis, therapy and complications are discussed.
