Abstract

Pretreatment with antihistamines has been claimed to reduce the adverse reactions in hymenoptera venom hyposensitization. In a prospective randomized double-blind placebo controlled study in 121 patients with honeybee or yellowjacket venom allergy we studied the efficacy of pretreatment with 120 mg terfenadine or 120 mg terfenadine + 300 mg ranitidine in hymenoptera venom rush hyposensitization. The incidence of systemic and local adverse reactions was evaluated for up to 50 weeks. Anaphylactoid reactions in 7 patients - 6 under premedication with placebo and 1 with terfenadine - demanded discontinuation of therapy (p < 0.05). After week 1 significantly fewer local reactions were recorded in patients treated with terfenadine and with a combination of terfenadine + ranitidine compared with subjects given placebo. These differences decreased during the course of hyposensitization. Premedication with H1 or H1 and H2 antihistamines significantly reduced systemic as well as local adverse reactions of hymenoptera venom hyposensitization. There was no difference between H1 and H1 plus H2 prophylaxis.
