Abstract
Profound changes in metabolism are a common problem after major surgery. Altered plasma concentrations of eicosanoids, cytokines and hormones reflect the metabolic stress leading to a catabolic state. In spite of the changes in lipid metabolism lipid emulsion especially a mixture of LCT/MCT in adequate amounts not exceeding 30-50 per cent of total energy are a suitable source of energy in this situation, n-6 and n-3 fatty acids are precursors of eicosanoids with different effects on several important functions partly via cytokines. Manipulation of the n-6/n-3 fatty acid ratio can lead to amplification or suppression of inflammatory reactions and immune response. Human studies indicate that eicosapentaenoic acid (n-3) reduces the response to pro-inflammatory stimuli probably resulting in increased survival of patients after major surgery.
