Abstract
A precise and reliable diagnostic work-up and staging is a precondition for multimodal strategies in the treatment of gastrointestinal tumors. Even in most advanced imaging procedures, however, the diagnostic accuracy is still limited, warranting to discuss the place of extended diagnostic laparoscopy again. According to current data, the impact of an additional diagnostic laparoscopy has to be considered differently for various tumor entities. In case of an advanced adenocarcinoma of the distal esophagus or in gastric cancer, additional information can be expected in as much as 20-40%. In those cases and in particular if multimodal therapy is considered, EDL is recommended. This is different in case of esophageal squamous cell carcinoma. Advanced pimary or secondary lesions of the liver seem to be good indications for EDL whereas its value in pancreatic cancer is doubtful.
