Abstract
Drug hypersensitivity reactions (DHR) may be severe, life-threatening, and even fatal [1]. Readministration of a culprit drug for DHR is the most important risk factor for recurrence, and may cause more severe and life-threatening reactions [2, 3]. Prescription errors are the reason of 70% of medication errors [4]. In most cases, lack of proper DH documentation is the cause of prescription errors [4-6]. Although unknown, drug hypersensitivity (DH) is believed to persist for years and can even be lifelong [1].
