Certain foods can provoke atopic eczema in some patients, but this is only one etiologic factor among others. In newborns with an elevated level of total serum IgE strict breast feeding seems to inhibit or to delay the manifestation of atopic disease. The pathomechanisms of provocation of eczema by foods have not yet been elucidated. Immediate type allergic reactions may cause eczema by provoking scratch responses or by the induction of late cutaneous reactions; furthermore, other allergic or non-immunologic (pseudo-allergic) mechanisms have to be considered, too. In order to identify an offending food, controlled double blind challenge tests should be performed. Skin testing and RAST may provide some information, but these procedures are not sufficient for a conclusive diagnosis. Evaluation of the effects of general 'exclusion diet' regimens on an outpatient basis is difficult. There is the risk of malnutrition and of anaphylactic reactions after re-introduction of allergenic foods. Dietary recommendations in atopic eczema should be based on a careful allergologic work-up of the individual patient.
