Abstract

From 1996 to 2000, 93 patients underwent BSR, 25 patients BVSR and 22 patients SP. After BSR and BVSR there were 42 patients with an uneventful postoperative course, whereas the remaining 74 patients developed minor or major complications resulting in re-operation in 19 cases including completion pneumonectomy in 12 patients. There were 4 postoperative deaths resulting from sepsis (n = 3) and bronchoarterial fistula. After a median follow-up period of 22,2 months the recurrence-free 1- and 3-year survival rate was 79,5% and 61,4%, respectively. Following SP, complications occurred in 16 patients; among them were 6 patients with re-operation for revision of anastomosis and related problems and 1 patient who died. Presently, still 20 patients are alive and 13 without evidence of recurrence. Sleeve resection including combined bronchovascular sleeve resection and sleeve pneumonectomy for lung cancer may be accomplished with acceptable mortality rates, however, careful patient selection and meticulous surgical technique as well as postoperative care remain the key to minimizing morbidity and mortality rates and achieve long-term survival that is comparable to corresponding tumor stages after standard procedures.
