Abstract

Autologous osteochondral transplantation currently represents one of the most important possibilities for the treatment of osteochondral defects. Using this technique, cartilage-bone plugs harvested from the proximal femoral condyle are used for autologous transplantation in nearly every joint. The authors, with experience of more than 400 osteochondral autograft transplantation cases since 1996, report the common technique of autologous osteochondral transplantation for the treatment of talar osteochondral defects and a new technique of a retrograde osteochondral autograft transplantation for the treatment of isolated osteochondral lesions of the distal tibia. Patients were followed up for 32 (12-74) months. A complete healing of the grafts was seen in control magnetic resonance images. All patients were satisfied with the surgery. The advantage of this method in comparison to bone marrow stimulating procedures is the restoration of the articular surface with hyaline cartilage of superior biological and biomechanical quality. The disadvantages such as the necessity of knee joint arthrotomy and malleolar osteotomy are, according to our data, in the long term without clinical relevance. When correctly indicated, autologous osteochondral transplantation offers an excellent alternative in the treatment of osteochondral defects of the ankle.
