Abstract
The majority of vascular dementias are caused by subcortical microinfarcts and ischemic white matter changes, not by cortical territorial and watershed infarcts. Therefore, the psychopathology of vascular dementias is characterised by impairment of executive function and attention, and personality change. On the other hand the typical course of multiinfarct dementia including an abrupt onset and a stepwise deterioration are rare. The repertoire of symptomatic treatments for vascular dementias has recently been expanded by the demonstration that cholinesterase inhibitors and NMDA receptor antagonists are effective in probable vascular dementia and Alzheimer's disease with cerebrovascular components.
