Abstract
The purpose of this prospective study was to compare the influence of the class-I antiarrhythmic drugs prajmaline and propafenone. 58 patients with high-grade symptomatic ventricular arrhythmias and underlying heart disease were given the two drugs in a randomized and placebo-controlled acute oral testing. Prajmaline and propafenone showed a comparable efficacy with 50% and 55%, respectively. Aggravation, calculated following the 'Munchner Modell', was noted in 5% of the patients using placebo, and in 7% when one of the two antiarrhythmic drugs had been administered. Side effects were significantly more frequent under both drugs than under placebo: prajmaline 19%, propafenone 22%.
