N2 - This chapter gives an overview of the clinical, diagnostic, and research use of transcranial magnetic stimulation (TMS) in children. First, we present aspects of normal developmental neurobiology while focusing on neuromotor development. TMS can be used to evaluate synaptic plasticity, the neuronal correlate of learning that is dependent on behavior and practice. During early childhood, the acquisition of motor skills is a process that goes in line with maturational changes to the central nervous system.TMS-evoked parameters that reflect aspects of neuromotor maturation are explained. In line with the various structural and functional changes to the developing brain, parameters like the threshold, latency, amplitude of a motor-evoked potential, and synaptic plasticity are observably changing with age furthermore, ongoing intracortical inhibition can be investigated. Alterations in TMS-evoked parameters are outlined for various neurodevelopmental disorders and an outlook is given.
